Langs is the sponsoring organization for the Community Diabetes Program, Waterloo Region, which provides education and support for people to self-manage Type 1, Type 2, gestational diabetes, as well as those who have pre-diabetes.

 

Patients become better equipped to make healthy lifestyle choices to control blood sugar and reduce the risk of chronic problems associated with diabetes.
Virtual, In-person, and hybrid options are available for appointments and classes!

  • Individual appointments with a Certified Diabetes Educator
  • Group Education on diabetes, insulin management, carb counting, heart health, label reading and glycemic control
  • Physical Activity Classes
  • Chiropody services and Foot Care
  • Retinal Screening (when available)
  • Support in Diabetes in Pregnancy including gestational diabetes and post-partum education
  • Specialist clinics, insulin pump management and support
  • Insulin pump start
  • Drop-in support groups

Who Do We Serve?

 

We serve adults with “borderline” or pre-diabetes, gestational diabetes, as well as children and adults with newly diagnosed or existing type 1 and type 2 diabetes. Family members or other support persons are welcome to attend your appointments with you.

 

How can I get started?

 

Find out if you are at risk of diabetes by attending a pre-diabetes education session or taking this online test.

 

To join our program, there are two options.

 

Option 1:

  • Get referred by your doctor or NP. If your doctor/NP refers you to our program, our receptionist will contact you to book an appointment.

Option 2:

  • Adults with diabetes or who are at risk for diabetes can self-refer to our program by completing our intake form online or calling 519-653-1470 x285.
