This service connects people to community services to support their physical and mental health needs based on the social determinants of health. 

 

What do Health Guides do?

 

Working with primary care providers, Health Guides provide short-term services to assist clients in navigating complex medical and social services.  ​

 

Guided by the client, we use an inclusive and flexible approach to:  ​

  • Consult with the referral source to discuss the client’s psychosocial, physical, and socioeconomic needs. ​
  • Explore and navigate community services to address clients’ social determinants of health aligning with clients’ well-being and autonomy. ​
  • Connect, refer, and advocate with clients to meet their goals and needs through case coordination. ​

 

Who would benefit from this service?

 

Referrals are sent by primary care, health, social service organizations, or self-referral

 

Health Guides assist clients who:

  • Have complex chronic conditions.
  • Are not managing well, even with frequent doctor visits or repeated calls for assistance.
  • Are visiting the emergency department or being hospitalized frequently.
  • Can not get to regular office appointments, for whatever reason.
  • Are not getting the care and support they need.

Connectivity Waterloo Region

Connectivity Waterloo Region is a unique approach to providing immediate, coordinated, and integrated responses, through the mobilization of resources to address situations of acutely elevated risk factors.

 

Langs Community Health Centre created one of the early formed situation tables in 2014 in Ontario, known as Cambridge North Dumfries (CND) Connectivity. To celebrate 10 years of Connectivity, CND amalgamated with Camino Well-being + Mental Health, KW4 table, with the new name Connectivity Waterloo Region.

 

Connectivity Waterloo Region brings together over 40 agencies from regional, medical, mental health, and social organizations to a weekly meeting. Members engage in a highly disciplined discussion that results in the identification of situations that place individuals at high risk of imminent harm. Here, agencies collaboratively plan and rapidly mobilize a response that quickly and efficiently connects those at risk with the services and supports they need.

 

When should a client or family (situation) be referred to Connectivity Waterloo Region? 

 

Any organization can bring or refer a situation to Connectivity. Connect with a Co-chair, or representative from your agency, to complete a referral. Situations brought to the meetings, by an organization, are those that involve:

  • High level of significant risk to an individual, the community, or both
  • Previous attempts to provide service have not been effective
  • Situations that could be more effectively managed in a multi-organization manner

Connectivity Waterloo Region Connectivity table works collaboratively through a Community of Practice, and we also work closely with the Ministry of Community Safety and Correctional Services, and the Office of the Information and Privacy Commissioner of Ontario to further guide the practice of the table.
