Langs provides comprehensive outreach support across multiple sites, offering primary care, social work, and assistance from dedicated outreach workers. These services include health care and system navigation to help individuals access health and community resources. Outreach is designed for people who are experiencing homelessness or insecure housing. This team is dedicated to ensuring equity and access while being innovative in their approach to care.

 

Homeless and Addiction Recovery Treatment Hub (HART)

 

The HART Hub Team provides care and coordinated support to meet immediate needs, and help people who are facing mental health, substance use and housing challenges in Waterloo Region. The Langs HART Hub team can be found at various locations in Cambridge.

Interprofessional Primary Care Team (IPC)

 

The IPC Team is a multidisciplinary group of healthcare professionals working together to provide comprehensive care to the community of Cambridge. The team provides outreach and healthcare to those who are unhoused, precariously housed or face other barriers to care. IPC provides a coordinated approach, working with other community partners at various community sites.
